Anti–SARS-CoV-2 monoclonal antibodies are effective at preventing progression to severe COVID-19 and as prophylactic agents (NEJM JW Infect Dis Nov 2021 and N Engl J Med 2021 Dec 2; 385:e81); but what about patients already hospitalized with COVID-19? In a platform trial at 127 U.K. hospitals, investigators randomized such patients to receive usual care with or without an infusion of casirivimab and imdevimab (4 g each). The primary endpoint was 28-day mortality. During the trial (but before unmasking of results), the analysis was revised to test the efficacy of this combination first in patients who were seronegative at baseline for anti–SARS-CoV-2 spike antibodies, and second in the overall study population.
