Despite advances in antiretroviral therapy coverage globally, cryptococcal meningitis continues to be a major cause of morbidity and mortality among people with HIV. Since 2018, the WHO has recommended a regimen consisting of 1 week of amphotericin B deoxycholate plus flucytosine followed by 1 week of high-dose fluconazole. However, the intravenous administration of 7 days of amphotericin remains challenging in many countries. In a trial in sub-Saharan Africa, Jarvis and colleagues tested the WHO-recommended regimen (control) against a single high-dose infusion of liposomal amphotericin B (10 mg/kg) followed by 2 weeks of oral flucytosine plus fluconazole.
