Long-term dual antiplatelet therapy (DAPT) with aspirin and clopidogrel is not recommended for secondary stroke prevention due to excess bleeding risk. However, short-term DAPT was shown to reduce early recurrent stroke risk after minor ischemic stroke and transient ischemic attack in the CHANCE and POINT trials, which were reported in 2013 and 2018, respectively. Accordingly, recommendations for short-term DAPT were incorporated in successive revisions of the American Heart Association/American Stroke Association guidelines with progressively higher level-of-evidence classifications from 2011 to 2019.
