Cefiderocol is a novel siderophore cephalosporin approved by the FDA for treating complicated urinary tract infections and hospital-acquired or ventilator-associated pneumonia caused by carbapenem-resistant Gram-negative bacteria. A randomized trial of cefiderocol versus best available therapy for patients with such infections (CREDIBLE CR) found unexpectedly higher mortality in the subset of patients with carbapenem-resistant Acinetobacter baumannii (CRAB) who received cefiderocol — and unexpectedly lower mortality in the control arm, contrary to real-world experience (as well as trials comparing the newer beta-lactam antibiotics with colistin).
