In the general critical care population, early tracheostomy may result in fewer intensive care (ICU) complications and improved functional outcomes. However, for patients with severe stroke, who are at high risk for requiring tracheostomy and for having poor outcomes but much lower risk for cardiopulmonary respiratory failure, the optimal timing of tracheostomy is unknown. A single-center, randomized pilot trial (SETPOINT) previously suggested that early tracheostomy may lower mortality and duration of ventilation for severe stroke. Now, the follow-up multicenter SETPOINT2 trial has evaluated whether early tracheostomy can potentially improve functional outcomes.
