In traumatic brain injury, acute subdural hematoma is the most prevalent focal lesion and is associated with high mortality and morbidity. Treatment is oriented to immediate surgical management or observation with a potential delayed surgery. In patients with rapid neurologic deterioration and increased intracranial pressure, the decision of surgery is clear. In some patients with subdural hematoma with similar characteristics, patients were treated surgically because of preferences according to centers.
