Periprosthetic joint infections (PJIs) are among the most common and problematic complications of total hip or knee arthroplasties. Leaving the joint implant in place is certainly preferred by surgeons and has led to the strategy of wound debridement, antibiotic therapy, and implant retention (DAIR). Although controlled trials have shown that early oral antibiotic therapy is noninferior to IV therapy for bone and joint infections (NEJM JW Infect Dis Mar 2019 and N Engl J Med 2019 Jan 31; 380:425) and 6 weeks of antibiotic therapy is not noninferior to 12 weeks (NEJM JW Infect Dis Jul 2021 and N Engl J Med 2021 May 27; 384:1991), the optimal approach to antibiotic therapy with DAIR remains unresolved.
