High CMV Viremia and Mortality in Hospitalized Patients with AIDS

In a retrospective study of patients (median CD4 count, 33 cells/mm³) at one hospital in China, 182-day mortality was associated with high CMV viral load among those not receiving effective ART.

Cytomegalovirus (CMV) infection has a long history in association with AIDS, starting with initial concerns about causation, significant contribution to AIDS-related morbidity and mortality, and linkage with ongoing inflammation. Now, investigators at a single hospital in China retrospectively analyzed 182-day mortality in HIV-infected adults admitted between 2017 and 2021; at this hospital, patients with CD4 counts <200 cells/mm³ had routine measurement of CMV DNA in peripheral blood leukocytes. CMV DNA levels were stratified into 3 levels: below the limit of quantification (<500 IU/mL), low (500–10,000 IU/mL), and high (>10,000 IU/mL).
