Several randomized trials published in 2022 addressed drug therapies for slowing progression of chronic kidney disease (CKD). Inhibitors of the renin-angiotensin system have long been used for this purpose, but when renal insufficiency becomes severe, clinicians often worry about the safety of continuing these drugs. To address this concern, researchers identified about 400 nondialyzed patients with advanced CKD (estimated glomerular filtration rate [GFR], <30 mL/minute/1.73 m2) who had been taking angiotensin-converting–enzyme (ACE) inhibitors or angiotensin-receptor blockers (ARBs).
