Weight gain has been linked to antiretroviral therapy (ART) containing second-generation integrase inhibitors (INSTIs) or tenofovir alafenamide (TAF), but the clinical significance remains uncertain (NEJM JW Infect Dis Jan 2020 and Clin Infect Dis 2019 Oct 14; [e-pub]). Now, two studies provide new insights. In a follow-up to the ADVANCE Study, researchers assessed the metabolic effects of switching from one of the assigned study regimens (TAF/FTC + DTG, TDF/FTC + DTG, or TDF/FTC/EFV) to TDF/3TC/DTG after 192 weeks. Analysis of data from 172 patients focusing on weight showed that women — but not men — who switched from TAF/FTC + DTG to TDF/3TC/DTG experienced significant weight reduction (−1.6 kg, P=0.013).
