Tenecteplase is a modified thrombolytic with greater specificity for fibrin and a longer half-life, which allows it to be administered as a single intravenous bolus rather than as a bolus and hour-long infusion as is required for alteplase. Recent noninferiority trials have supported the noninferiority of tenecteplase compared with alteplase and clarified that the 0.25-mg/kg dose is preferred, but some residual uncertainty has remained about the appropriate dose for certain Asian populations, given that lower doses of alteplase (0.6 mg/kg) are used in some Asian countries, such as Japan.
