Reconsidering CMV Serostatus in Hematopoietic Cell Transplant Recipients

A single-site study suggests previously CMV seronegative HCT recipients who are weakly CMV seropositive with undetectable viremia pretransplant may be safely reclassified as CMV seronegative.

Cytomegalovirus (CMV) serostatus prior to hematopoietic cell transplant (HCT) has implications for posttransplant CMV monitoring and prophylaxis as well as donor choice. Receipt of numerous transfusions between diagnosis and HCT can yield false-positive CMV serology, with some CMV seronegative patients being erroneously classified as seropositive.
