Misdiagnosis of multiple sclerosis (MS) remains a problem (NEJM JW Neurol 2021 Mar 2 and Mult Scler 2021; 27:913; NEJM JW Neurol 2023 Sep 11 and Lancet Neurol 2023; 22:750). Cortical lesions and central vein sign (CVS) offer greater specificity for MS among those with white-matter lesions (NEJM JW Neurol 2018 Feb 8 and Ann Neurol 2018; 83:283).
