Cortical Lesions and Central Vein Sign in Multiple Sclerosis

These measurements have diagnostic value for excluding MS mimics.

Misdiagnosis of multiple sclerosis (MS) remains a problem (NEJM JW Neurol 2021 Mar 2 and Mult Scler 2021; 27:913; NEJM JW Neurol 2023 Sep 11 and Lancet Neurol 2023; 22:750). Cortical lesions and central vein sign (CVS) offer greater specificity for MS among those with white-matter lesions (NEJM JW Neurol 2018 Feb 8 and Ann Neurol 2018; 83:283). To examine the value of these two MRI biomarkers in MS diagnosis, researchers conducted an international, multicenter, retrospective analysis of brain MRI scans collected from 2010 to May 2020 from 599 patients with MS and 452 with non-MS conditions.
