Uptake of PrEP among U.S. persons at risk for HIV infection remains disappointingly low as out-of-pocket costs threaten access. To determine the impact of copays on initial PrEP fill rates among persons with insurance-approved PrEP, investigators queried a proprietary database containing claims for 80%–85% of all PrEP prescriptions in the U.S. from 2015 to 2019. Initial PrEP fill rates were determined across 6 out-of-pocket cost categories ranging from $0 to >$500, and a model was developed to simulate changes in such expenses and rates of subsequent diagnoses of HIV infection relative to those expenses.
