Long-Acting Cabotegravir and Rilpivirine for Treatment of Persons with HIV: Updated Recommendations

Newly revised recommendations support initiation of long-acting injectable cabotegravir/rilpivirine (LA CAB/RPV) for selected PWH who are not virologically suppressed.

Background

Since the last update of the IAS–USA treatment guidelines in December 2022, experience with long-acting injectable cabotegravir/rilpivirine (LA CAB/RPV) for treating patients without virologic suppression has grown. In addition to several published case series, modeling studies have shown that the benefit of LA CAB/RPV is greatest in those with advanced HIV who have difficulty adhering to oral therapy.

Key Recommendations

The IAS–USA guidelines panel has issued updated recommendations for use of LA CAB/RPV in PWH without virologic suppression who meet the following additional criteria:

Inability to take oral ART consistently despite optimal clinical support
