Overall stroke mortality has steadily improved in recent decades. Still, racial and ethnic disparities in stroke outcomes and recurrence remain unsettlingly persistent, in large part driven by the differing impacts of vascular risk factors, especially hypertension. Multidisciplinary approaches targeting blood-pressure (BP) control with patient monitoring and self-management have generally shown promise, but whether this approach would prove helpful for low-income Black and Hispanic patients is unclear. Now, in a practice-based, randomized trial at eight health centers in New York City, investigators compare the effects of home BP telemonitoring (HBPTM) alone or with telephone-based nurse case management in this high-risk population.
