The role of inflammation in stroke pathogenesis has received increased attention in the past decade. Colchicine affects several mediators of inflammation, including proinflammatory cytokines. It has a demonstrated benefit in secondary prevention of ischemic heart disease but has not been tested in ischemic stroke, which can be caused by any of several mechanisms. These investigators conducted a randomized, open-label, blinded-endpoint trial in which patients with a noncardioembolic stroke (in the preceding 3–28 days) were assigned to colchicine (0.5 mg per day) plus usual care or usual care alone. The primary endpoint was stroke, myocardial infarction, cardiac arrest, or hospitalization for unstable angina.
