Physicians are often uncomfortable prescribing full-dose anticoagulation for patients over the age of 80 because of increased risk for bleeding. But can a reduced dose still prevent stroke in older patients with atrial fibrillation (AF)? To examine this question, researchers conducted a post hoc analysis of data from the ENGAGE AF TIMI 48 trial, comparing outcomes in patients aged ≥80 randomized to a higher dose (60 mg) or lower dose (30 mg) of edoxaban or warfarin. In the higher-dose edoxaban group, the assigned dose was reduced from 60 to 30 mg if patients had low creatinine clearance or body weight or were taking strong P-glycoprotein (permeability glycoprotein) inhibitors.
