A principal concern with intravenous thrombolysis for acute ischemic stroke is bleeding risk. Therefore, patients who have recently taken an oral anticoagulant typically have been deemed ineligible for treatment. The development of reversal agents such as idarucizumab, a monoclonal antibody that binds to the direct thrombin inhibitor dabigatran to reverse its anticoagulant effect, has raised interest in whether reversing anticoagulation in order to then administer intravenous thrombolysis might be a viable strategy.
