Evidence Mounts for Triple Single-Pill Combination Therapy for Hypertension

A trio of trials provide more evidence for the safety and efficacy of SPCs to lower blood pressure.

For adults with stage 2 hypertension whose blood pressure (BP) is above goal by >20/10 mm Hg, an American College of Cardiology/American Heart Association guideline recommends initial therapy with two first-line agents of different classes, either as separate agents or in a fixed-dose combination (NEJM JW Gen Med Dec 15 2017 and J Am Coll Cardiol 2018; 71:127). Many patients with hypertension will require more than one medication to achieve BP control; in such patients, using single-pill combination (SPC) therapy might improve adherence and control BP more quickly. Moreover, combining drugs from different classes can be synergistic.
