Urological organizations recommend against screening for prostate cancer in men with limited life expectancy (NEJM JW Gen Med Aug 1 2023 and J Urol 2023; 210:46). When prostate cancer is detected in these patients, guidelines recommend active surveillance or watchful waiting over definitive treatment (i.e., surgery or radiotherapy) in many cases. To assess whether clinical practice reflects these recommendations, researchers identified a cohort of ≈244,000 U.S. veterans with clinically localized prostate cancer (mean age at diagnosis, 67) and used a validated score to estimate life expectancy.
