Studies published in the past year informed us about the optimal duration of dual antiplatelet therapy (DAPT), which is indicated for many patients who suffer thrombotic strokes, and about when anticoagulation should be initiated after thrombotic or embolic strokes.
Patients with minor thrombotic strokes or high-risk transient ischemic attacks (TIAs) benefit from DAPT when it is initiated within 24 hours of stroke onset (NEJM JW Gen Med Jul 1 2018 and N Engl J Med 2018; 379:215), but patients who presented >24 hours after symptom onset have not been included in previous trials.
