Although the MINT trial in 2023 suggested that anemic patients with myocardial infarction (MI) — and particularly type 1 MI — might benefit from a hemoglobin transfusion threshold as high as 10 g/dL, an additional analysis from that trial in 2024 suggests that a transfusion threshold of 9 g/dL likely confers a survival benefit similar to a threshold of 10 g/dL. Patients with type 1 acute MI likely deserve a transfusion threshold that is more liberal than 7 or 8 g/dL; a threshold of 9 g/dL might be optimal (NEJM JW Gen Med Nov 15 2024 and Ann Intern Med 2024; 177:1489).
