Sodium–glucose cotransporter-2 (SGLT-2) inhibitors reduce hospitalizations related to heart failure (HF) in high-risk patients. To assess the efficacy of SGLT-2 inhibition in patients with HF and valvular disease, researchers randomized patients with HF and aortic stenosis undergoing transcatheter aortic valve replacement (TAVR) in Spain to receive dapagliflozin (10 mg/day) or standard care alone, in open-label fashion (NCT04696185).
The primary analysis included 1222 patients (mean age, 82 years; 49% women; mean pre-TAVR aortic valve gradient, 47 mm Hg); 44% had diabetes, 88% moderate renal insufficiency, and 17% left-ventricular systolic dysfunction.
