Although radiation therapy after breast-conserving surgery lowers risk for local recurrence in early breast cancer, it might not meaningfully affect cancer-specific or overall survival in older low-risk women according to previous clinical trials (NEJM JW Gen Med Mar 15 2023 and N Engl J Med 2023; 388: 585). Now, a planned cohort study provides more data on outcomes in low-risk breast cancer treated without radiation. Researchers followed 601 women (age, ≥65) who had small (≤2 cm), hormone receptor–positive tumors surgically removed with clear margins and no lymph node involvement. All patients received 5 years of adjuvant endocrine therapy.
