On July 14, 2025, the U.S. FDA approved use of finerenone (Kerendia) “to reduce the risk of … cardiovascular death, hospitalization for heart failure, and urgent heart failure visits in adult patients with heart failure with left ventricular ejection fraction ≥40%.”
Finerenone is a nonsteroidal mineralocorticoid-receptor antagonist; it was approved in 2021 for patients with type 2 diabetes and chronic kidney disease after studies showed that it lowered the incidence of adverse renal and cardiovascular outcomes in those patients. However, its efficacy in patients with heart failure and preserved (or mildly reduced) ejection fraction had not been established.
