In patients who are intubated, self-extubation can lead to reintubation and extended ICU stays. What factors predict reintubation, and how does reintubation affect outcomes? To address this, researchers reviewed data from 13,000 intubated patients in French ICUs during a 25-year period. Those with limitations on goals of care were excluded from analysis.
More than 700 patients self-extubated during their ICU stay, and 40% were reintubated within 48 hours. Predictors of reintubation included:
An admission diagnosis of respiratory failure or chronic obstructive pulmonary disease
A higher degree of organ failure the day before self-extubation
Prolonged duration (>7 days) of mechanical ventilation before self-extubation
