Most U.S. hospitals have triage systems to identify and manage patients with out-of-hospital (OOH) ST-segment myocardial infarction (STEMI). Yet few have protocols for in-hospital (IH) STEMI. To compare patient characteristics and clinical outcomes between OOH and IH presentation, researchers assessed data from more than 100,000 patients with acute MI (96% OOH, 4% IH) from a nationally representative registry of 670 hospitals from 2019 through September 2022.
Patients with IH presentation were significantly older (median age, 67 vs. 63 years) and more likely to have risk factors including diabetes, heart failure, and prior MI than patients with OOH MI.
