Both hereditary and wild-type transthyretin amyloidosis (ATTR) are associated with an increased risk of cardiomyopathy but may also affect other organs and tissues. Polyneuropathy is a well-established feature of hereditary ATTR, but how common is it in wild-type ATTR? To find out, researchers at an Italian referral center prospectively studied 30 patients with wild-type ATTR cardiomyopathy and no preexisting conditions known to cause polyneuropathy (mean age, 81; 80% men). All patients underwent neurologic examination and sural nerve conduction studies; sural nerve biopsy was offered if neuropathy was present.
Carpal tunnel syndrome was noted in 70% of patients; most cases were bilateral.
