It is difficult to unsee motile roundworms on a slit-lamp examination. Thelaziasis is a zoonotic nematode infection transmitted by drosophilid flies that feed on ocular secretions. Manifestations range from lacrimation, photophobia, and conjunctivitis to keratitis, corneal ulceration, and, rarely, blindness. Diagnosis is made by visualizing the worms in the conjunctival sac or lacrimal secretions, with species identification through polymerase chain reaction. Treatment involves mechanical removal of the worms from the conjunctival sac. Seeing is believing — watch the accompanying video to see the wriggling in action.
