The American Heart Association and American College of Cardiology — along with other societies — published a new hypertension guideline in 2025. Their 2017 guideline had redefined hypertension as blood pressure (BP) >130/>80 mm Hg (>140/>90 mm Hg was the previous standard) and recommended 130/80 mm Hg as a target in certain high-risk patients. However, subsequent research has only strengthened the case for lower BP targets.
Previously, intensive BP control was recommended only for patients with 10-year cardiovascular risk of >10%. The new guideline promotes earlier and more aggressive treatment of nearly all patients with hypertension and recommends a target BP of <130/<80 mm Hg, “with encouragement” to achieve systolic BP <120 mm Hg.
