In these early days of the U.S. monkeypox (MPX) outbreak, modes of transmission, clinical presentations, and responses to therapy remain to be characterized. Now, a CDC report chronicles diagnosis, treatment, and outcomes in five patients (age range, 20–39), two of whom were HIV positive and immunocompromised. During the course of generalized MPX infection, all developed lesions of the eye that were probably or definitely caused by MPX virus.
MPX-associated ocular disease took the form of keratoconjunctivitis and blepharitis similar to that caused by herpes simplex virus; indeed, four patients received trifluridine (an anti–herpes virus drug), indicating initial confusion about the etiology of the ocular findings.
