These authors report an interesting case of an otherwise healthy asymptomatic 5-year-old boy who had multiple episodes of white or cloudy urine during an 18-month period. The mother reported a white chalky precipitate on the boy's underpants when the cloudy urine dried. Initial laboratory evaluations of blood and urine (including protein) were normal except for an elevated urine calcium-to-creatinine ratio of 0.36 mg/mg (normal value is <0.2 mg/mg). A layer of white powdery precipitate settled at the bottom of a urine sample after it sat for 30 minutes.
