Atazanavir-based regimens are among the guideline-recommended combinations for initial antiretroviral therapy. Although generally safe, atazanavir has been associated with both hyperbilirubinemia and nephrolithiasis (JW AIDS Clin Care Aug 22 2011). Recently, two cases of atazanavir-associated cholelithiasis were reported.
Now, investigators have described an additional 14 patients who developed complicated cholelithiasis (cholecystitis in 11, cholangitis in 1, pancreatitis in 4) while receiving an atazanavir-based regimen through a network in western France. During the study period (2006–2012), about 5000 HIV-infected patients were treated within this network; 20% of them were receiving atazanavir.
